Influenţa stress-ului oxidativ asupra cardiomiocitelor în insuficienţa cardiacă congestivă indusă prin ischemie miocardică ateromatoasă

Rezumat

În baza datelor din literatura de specialitate consultată, referitoare la modul în care stress-ul oxidativ intervine în patogenia insuficienței cardiace congestive (ICC),  prezenta lucrare sintetizează modul în care este redusă masa miocardică contractilă, componenta patogenică principală a deficitului de pompă aspiro-respingătoare a inimii. Reducerea contractilităţii miocardului, în condiţii de stress oxidativ indus ischemic recunoaşte un dublu mecanim: exacerbarea procesului de moarte prematură a cardiomiocitelor (prin necroză şi apoptoză) şi alterarea calitativă a organitelor cardiomiocitelor care supravieţuiesc stress-ului oxidativ existent în miocard şi care funcţional sunt suprasolicitate. Dintre cele două modalităţi de afectare numerică a cardiomiocite-lor este prezentat mecanismul apoptozei induse prematur şi la cote crescute.

Influence of oxidative stress on cardiomyocytes in congestive heart failure induced by atheromatous myocardial ischemia

Abstract

Based on the data from the consulted specialized literature, regarding the way in which oxidative stress intervenes in the pathogenesis of congestive heart failure (CHF), the paper presentation synthesizes the way in which the myocardial contractile mass is reduced, the main pathogenic component of the heart’s suction-repulsion pump deficiency. The reduction of myocardial contractility, under conditions of ischemic-induced oxidative stress recognizes a double mechanism: the exacerbation of the premature death process of cardiomyocytes (through necrosis and apoptosis) and the qualitative alteration of cardiomyocyte organelles that survive oxidative stress in the myocardium and that are functionally overloaded. Among the two ways of numerical damage to cardiomyocytes, the mechanism of apoptosis induced prematurely and at elevated levels is presented.
